PARTICIPANT WAIVER & RELEASE OF LIABILITY
Assumption of Risk
I understand that indoor golf involves the use of real golf clubs and balls, swinging motions, and physical activity that can cause injury or property damage. Risks include, but are not limited to, being struck by a club or ball, slips, trips, falls, strained muscles, and equipment failure.
Release of Liability
To the fullest extent permitted by New Jersey law, I release and hold harmless Bayshore Golf Club, LLC, its owners, members, employees, and agents from any and all claims, demands, or causes of action for injury, death, or property damage arising out of my presence in or use of the facility, except to the extent caused by gross negligence or willful misconduct.
Responsibility for My Actions
I am responsible for any damage to simulators, balls, clubs, TVs, furniture, or other property caused by my careless, reckless, or intentional actions, or by those in my group that I am supervising.
Safety Rules
I agree to follow all posted rules and all instructions from Bayshore Golf Club staff, including rules regarding swing areas, clean shoes and clubs, number of people in a bay, alcohol use, and safe conduct.
Photos & Video
I understand that the facility may use photos or video taken on site for marketing and social media. If I do not wish to appear in such content, I will notify Bayshore Golf Club in writing at

Minors
If I am signing on behalf of a minor, I represent that I am that minor’s parent or legal guardian and that I am agreeing to this waiver on their behalf.
Binding Agreement
This waiver is governed by the laws of the State of New Jersey. I agree that my electronic signature has the same force and effect as an ink signature. By submitting this form, I confirm that I have read, understand, and agree to this Participant Waiver & Release of Liability.
